To support evolving documentation requirements for minors traveling internationally without a parent or legal guardian, ACIS and Encore Tours have created a standard Parental Authorization Form for all destinations. Parents or guardians must complete and upload this form before departure. The steps below explain how to download, complete, and submit the required materials.
Steps to complete the authorization process
- Download, sign, and complete the authorization form
– Download the appropriate form from the table below - Scan the completed form
– Be sure the signed form is clear, readable, and that all fields are filled out. - Attach a copy of legal identification
– Parents or guardians must include a copy of their legal ID, such as a passport or driver’s license. - Upload both documents
– Use the submission portal below to upload your completed form and ID. - Submit
